The second ballot isn't created until the state changes to IESG Review. But that state change currently doesn't happen until two days before the telechat. The AD should be able to create the second ballot and allow the IESG to begin balloting while the state is still External Review -- it would be up to the AD to decide whether it's appropriate to accept ballots at that time or not, depending upon the working group in question.
